Welcome to the Primary Children's Hospital Comprehensive Care Program, serving children aged 0 to 21 with medical complexities.

The primary goal of our program is to provide comprehensive care for kids who have more than three organ systems affected and require involvement from over three physicians. This population often experiences frequent or prolonged hospital stays due to their medical fragility and may also rely on technology-depencies, such as ventilators, tracheostomies, feeding tubes, intrathecal baclofen pumps, and other advanced care needs.

Our services cater to a wide range of conditions including but not limited to:

Premature birth

Congenital anomalies

Genetic syndromes

Cerebral palsy

Brn injuries

Spinal cord injuries

Neuromuscular disorders

Functional limitations requiring adaptive equipment

We define medical complexity as having more than three organ systems affected and over three physicians involved in their care.

Our multidisciplinary team works closely with pediatricians, subspecialists, hospitalists, discharge planners, and other healthcare professionals to ensure that your child receives the most appropriate care at the right time. Our services include:

Outpatient evaluations

Medical management

Care coordination

All these are performed in collaboration with your primary care providers.

For inpatients, we provide specialized consultations for complex management issues or medical dispositions. We're here to support and collaborate on decisions that impact every aspect of your child's healthcare journey, from acute hospitalizations to long-term management plans at home, school, and within the community.

We offer educational resources tlored specifically for parents, caregivers, and healthcare providers alike, to empower you with knowledge about various conditions and treatment options. Our team also coordinates referrals and manages medical appointments, advocates on your behalf with insurance companies, schools, government programs, and other service organizations, and facilitates seamless integration of care across different settings like hospitals, homes, community centers, etc.

We are dedicated to helping families understand complex diagnoses, discuss -of-life decisions for children with life-limiting conditions, guide through the transition process as kids grow up, and handle guardianship issues for adolescents moving towards adulthood.
