INTRODUCTION AND OVERVIEW OF THE PEDIATRIC COMPLEX CARE PROGRAM

The Pediatric Comprehensive Care program is a specialized support system designed for children aged from birth to 21 years who are experiencing complex health issues. A child's medical complexity is defined by having more than three organ systems affected and involvement of over three physicians in their care management, along with frequent hospitalizations due to medical fragility depencies such as ventilators, tracheostomies, feeding tubes, intrathecal baclofen pumps, among others.

Our program encompasses a broad spectrum of complex conditions including:

• Prematurity

• Congenital anomalies

• Genetic syndromes

• Cerebral palsy

• Brn injuries

• Spinal cord injuries

• Neuromuscular disorders

• Functional limitations requiring adaptive equipment

2. Medical Management and Care Coordination: Our team works with your primary care providers PCPs, subspecialists, hospitalists, and discharge planners to ensure seamless care coordination for your child.

3. Inpatient Consultation: For children with complex management issues, we offer consultations that involve interdisciplinary care conferencing as needed.

ADDITIONAL SUPPORT SERVICES

Our program also offers:

• Educational resources for parents and healthcare providers

• Coordination of referrals to community services, schools and other vital support systems

• Advocacy efforts on behalf of your child in dealing with insurance companies, ensuring appropriate coverage.

• Integration of pediatric care across various settings including hospitals, communities, homes.

• Guidance through decision-making processes related to children's life-limiting conditions.

• Support for transition plans as your child approaches adulthood and guardianship issues.
