1. Complex Medical Conditions: Two or more chronic conditions requiring active management.

2. Specialist Involvement: Care involves at least three specialists.

3. Technology Depency: May require private duty nursing, ventilator support, or other medical equipment.

4. Recent Hospitalizations: One or more admissions within the past two years andor one emergency room visit.
