We m to achieve a balanced approach that focuses on providing the 'right care' at 'the right place', and 'at the right time'. Our mission is to optimize healthcare for children with complex medical conditions, building strong partnerships with their caregivers and diverse medical teams. The goal is to enable these children to reach their full potential and live life more fully.

Care Coordination Program

This program ensures centralized care coordination by integrating multiple specialties across Children's Health network, ming at improving quality of life and community integration for affected children and families. We strive to provide an exemplary model that unifies the healthcare community around children with complex care needs.
